The Steps To The Treatment Of Abnormal Bleeding

Do you have persistent mid-period spotting, heavy periods, short cycles, or extra-long cycles? Between 10 and 30 percent of women experience abnormal bleeding during their reproductive years. If you're one of the many women experiencing this type of menstrual issue, take a look at what you need to know about the treatment of abnormal bleeding.

Find the Cause

Diagnosis is the first step towards treatment. Before you can find the right abnormal bleeding treatment, you need to visit the OBGYN's office. Your medical provider will ask you about the symptoms, your menstrual and medical history, and any other potentially related issues (such as lifestyle changes, birth control, or pregnancies). 

The gynecologist may also need to examine your cervix, uterus, and ovaries. If an obvious cause isn't apparent during the pelvic examination or the doctor suspects/wants to rule out specific issues, they may recommend an ultrasound, blood tests, a thyroid test, a pregnancy test, a hysteroscopic exam, or an endometrial biopsy. The specific tests the doctor orders depend on your symptoms, family/genetic history, the extent of the abnormal bleeding, and other related conditions.

Get a Diagnosis

Only a qualified medical provider can diagnose the cause of abnormal uterine bleeding. Even though this issue is common, it's isn't something you should self-diagnose. After the gynecologist diagnoses the cause, they can review the treatment options. 

Common causes of abnormal uterine bleeding include perimenopause (the years leading to menopause), hormonal changes, pregnancy, ovulation, fibroids, polyps, polycystic ovary syndrome (PCOS), birth control, infection/sexually transmitted diseases, or (less commonly, but still possibly) uterine or cervical cancer. 

Learn About Treatment Options

What are the available treatments for women in their reproductive years? There isn't one universal treatment for abnormal uterine bleeding. Instead, your medical provider will need to match the treatment to the cause or suspected cause. Treatments for this type of bleeding can include:

  • Hormonal birth control. Birth control pills, a hormonal intrauterine device (IUD), or other similar contraceptive options can help to control abnormal bleeding, regulate your periods, or eliminate periods completely.

  • Gonadotropin-releasing hormone agonists. Also known as GnRH, this type of medication is used to treat uterine fibroids (if treatment is necessary) and to stop periods.

  • Antibiotics. If you have an infection, an antibiotic can treat the problem and stop the resulting bleeding.

  • Other medications. Tranexamic acid and nonsteroidal anti-inflammatory drugs (NSAIDs) may help to alleviate heavy bleeding.

  • Surgical options. Endometrial ablation, uterine artery embolization, and hysterectomy can stop abnormal bleeding (or all bleeding) when medications don't work.

Some causes of abnormal bleeding may not require a specific treatment. Perimenopause can cause temporary hormone shifts that may result in spotting or period changes. The abnormal bleeding associated with this life change may resolve on its own. If it doesn't, or you have excessive bleeding, pain, or any other concerning symptom, contact the OBGYN immediately. Hormone replacement, natural therapies, and some types of birth control can alleviate perimenopausal abnormal bleeding for some women.
